The subject of his trauma is usually left in obscurity. Frequent smiles and good humor deter anyone from prying too deeply. It's a performative tactic, a forgery of the version of Gren that existed before war and prison. Perceptive characters that get closer to him may pick up on how he overcompensates, almost imitating someone who's self-assured rather than being genuinely secure with himself. He was at one time, but he isn't anymore.
He's severely depressed. The hormonal imbalance from the abuse of an experimental drug has had lasting effects on his mood, and it's bound to shift at times. When it does, he's likely to detach and remove himself from a situation. His instability has fostered self-destructive thoughts and tendencies, hints of these may crop up in introspection, vague dialogue, and β to a lesser extent β actions.
It was subtle in canonβ his words muddled in their respective contexts. This isn't a man who wanted to live. The bounty on his head had doubled due to the statute of limitation nearing its expiration. This would have granted him more freedom to possibly return home to old friends and family, but he prioritized his obsession with Vicious' betrayal. He wanted to face the truth even though he knew it wasn't one he wanted to live with.

"Either way, I don't have long to live."
"I'm not gonna make it anyway."
